After you have received a quote, if you want to see if your annuity qualifies for AFC's program, complete the attached Application and send it by email or fax to AFC.  There is no charge for submitting your Application and you are under no obligation to sell your annuity.  The Application allows AFC to determine whether AFC wants to purchase your annuity.  If AFC decides to purchase your annuity, AFC's attorneys will use the information on the Application to prepare the necessary court papers.  If AFC decides not to purchase your annuity, the Application and all attachments are destroyed.

IMPORTANT:  Attaching the following documents to your Application will help us evaluate your annuity more quickly:

  • Annuity Contract
  • Settlement Agreement / Court Judgment
  • Copy of your most recent Annuity Check or Check Stub.  If the payment is direct deposited, attach a copy of your bank statement (with statement number deleted)
  • Copy of your driver's license
  • Copy of your divorce decree (if you were divorced after the settlement occurred)
